Physical Examination: I will take a post-offer physical examination, from a physician designated by Sabre Yachts, at Sabre
Yachts’ expense. I authorize the physician or testing facilities to provide Sabre Yachts the results of the examination and I release Sabre from any liability from the use of such information for employment purposes. I understand that further employment
consideration for job placement may be affected by the results of the examination.
I authorize Sabre Yachts to conduct a criminal record and/or motor vehicle background investigation, and understand that this
investigative process will be at Sabre’s expense. I understand that this may include social security number verification and address history, criminal history, driving history, past employment information, reference checks and/or any other public records. I
authorize the complete release of these records. Such verification shall not constitute a violation of my right to privacy in any manner and I hereby release Sabre from all liability whatsoever for actions related to this information. I understand that the sole
purpose of obtaining this information is for employment reasons. I understand that I must provide my date of birth to adequately complete the background investigation, and acknowledge that my date of birth will not affect any hiring decision.
